Roots CFC Clinic
Roots Community Food Centre is hosting ID Clinics to help people obtain Birth Certificates and ONWA will be present to help with Status Card applications during the clinics.
We are only doing walk-ins (no appointments)
What you’ll need for birth certificate: date of birth, place of birth, legal name at birth & any name changes for you and your birth parents
What you’ll need for status applications: New registrations (long-form birth certificate + government ID required), Renewals (valid government photo ID required).

Birth Certificates: 10:30am-4:30pm (last intake will be at 4pm)
Status Applications: 10:30am-3:00pm (last intake will be at 2:30pm)
